Perinatal distress was measured using the EPDS (scores of 10–12) and probable perinatal depression was measured using the EPDS (scores of 13 or more). Logistic regression models that adjusted for confounding variables were used to investigate a range of psychosocial and obstetric determinants and perinatal distress and depression. Results: Eight percent of this cohort experienced antenatal distress and about 5 % experienced postnatal distress. Approximately 6 % experienced probable antenatal depression and 3 % experienced probable postnatal depression. Being from a culturally and linguistically diverse background (AOR = 2.0, 95% CI 1.8–2.3, P< 0.001), a lack of partner support (AOR = 2.9, 95% CI 2.3–3.7) and a maternal history of childhood abuse (AOR = 1.9, 95% CI 1.6–2.3) were associated with antenatal distress. These associations were similar in women with probable antenatal depression. Women who scored 10 to12 on antenatal EPDS assessment had a 4.5 times higher odds (95% CI 3.4–5.9, P< 0.001) of experiencing probable postnatal depression compared with women scoring 9 or less. Conclusion: Antenatal distress is more common than antenatal depressive symptoms and postnatal distress or depression. Antenatal maternal distress was associated with probable postnatal depression. Scale properties of the EPDS allows risk-stratification of women in the antenatal period, and earlier intervention with preventively focused programs. The arms included (1) BBL-GP, a web-based intervention augmented with an in-person diet and physical activity consultation; (2) a single clinician-led group, Lifestyle Modification Program (LMP); and (3) a web-based control. The primary outcome was the Australian National University Alzheimer Disease Risk Index Short Form (ANU-ADRI-SF). Results: Baseline assessments were conducted on 128 participants. A total of 125 participants were randomized to 3 groups (BBL-GP=42, LMP=41, and control=42). At immediate, week 18, week 36, and week 62 follow-ups, the completion rates were 43% (18/42), 57% (24/42), 48% (20/42), and 48% (20/42), respectively, for the BBL-GP group; 71% (29/41), 68% (28/41), 68% (28/41), and 51% (21/41), respectively, for the LMP group; and 62% (26/42), 69% (29/42), 60% (25/42), and 60% (25/42), respectively, for the control group. The primary outcome of the ANU-ADRI-SF score was lower for the BBL-GP group than the control group at all follow-ups. These comparisons were all significant at the 5% level for estimates adjusted for baseline differences (immediate: difference in means -3.86, 95% CI -6.81 to -0.90, P=.01; week 18: difference in means -4.05, 95% CI -6.81 to -1.28, P<.001; week 36: difference in means -4.99, 95% CI -8.04 to -1.94, P<.001; and week 62: difference in means -4.62, 95% CI -7.62 to -1.62, P<.001). Conclusions: A web-based multidomain dementia risk reduction program augmented with allied health consultations administered within the general practice context can reduce dementia risk exposure for at least 15 months. Types of intervention(s)/phenomena of interest: Intervention programs were required to have a parental component, to target eating and/or exercise behaviors for the prevention or treatment of obesity, and child weight status reported. Types of studies: All intervention studies were included in the review including: randomized controlled trials (RCTs); non-randomized controlled trials, longitudinal studies, cohort (both retrospective and prospective), case control and time series studies which had been conducted in a community setting. Types of outcomes: This review considered studies that included overweight/obesity related outcomes, eating behavior outcomes, physical activity behaviour outcomes, and sedentary behavior outcomes and constructs. Search strategy: A literature search of community-based parent-centred intervention studies to promote nutrition and physical activity for the prevention and treatment and of childhood overweight and obesity was performed in eight electronic databases dating from 1975 to April 2009. Methodological quality: Studies were critically appraised for methodological quality using standardized tools. Data collection: Data was extracted by one reviewer using a standardized data extraction form developed by the researchers and checked for accuracy and consistency by a second reviewer. Data synthesis: Data in relation to setting, methodology, intervention components and effect on weight, dietary intake and physical activity was extracted, and described in a narrative synthesis. Where possible a meta-analysis was undertaken. Results: Of the home-based interventions, five of nine studies reported statistically significant changes in anthropometric outcomes post intervention. Of the 10 studies in the before and after school care setting, seven reported a significant decrease in a weight-related outcome post intervention, with only one reporting an increase. Results: Despite the decreasing mortality rates projected for the period 2015–2019 to 2040–2044 for both men and women, the overall number of smoking-related cancer deaths is estimated to increase by 28.7% for men and 35.8% for women: from 138 707 (77 839 men and 60 868 women) in 2015–2019 to 182 819 (100 153 men and 82 666 women) in 2040–2044. Over the period 2020–2044, there will be 254 583 cancer deaths (173 943 men and 80 640 women) directly attributable to smoking, with lung, larynx, oesophagus and oral (comprising lip, oral cavity and pharynx) cancers having the largest SAFs. Interpretation: Cigarette smoking will cause over 250 000 cancer deaths in Australia from 2020 to 2044. Variation in projected absolute lifetime risk of breast cancer associated with classical risk factors was greater for women with higher genetic risk (PRS313 and family history) and, on average, 17.5% higher in the highest vs lowest deciles of genetic risk.
